Co-developed by Dr. Stephen White and Dr. Reid Meloy, renowned experts in the field of threat assessment, WAVR-21™ has been rigorously researched and tested to identify the key risk factors behind violence, risk, and threats.

Fully integrated within TIPS®, WAVR-21 V3 includes the Intake and Documentation Questionnaire, the Worksheet, the Grid, a User’s Guide, and the third edition manual.

The WAVR-21 – Workplace Assessment of Violence Risk – is a 21-item coded instrument for the structured assessment of workplace and campus targeted violence risk. First published in 2007 by its co-developers, Drs. Stephen White and Reid Meloy, the WAVR-21 reflects the authors’ extensive case and forensic experience and a thorough review of the research and clinical literature. The WAVR is an example of an SPJ (structured professional judgment guide) and is now in its third edition, published in 2016. SPJs are used in combination with practitioners’ professional or clinical judgment to objectively develop opinions of risk that inform challenging case management decisions.
